Existing Memberships: a Renewal Notice is sent just prior to the annual
expiration of BCHC Memberships. Returning the Renewal Notice with annual dues attached also provides members
an opportunity to update any information in our records.
In the event there is need for a separate (duplicate) form for RENEWING your membership,
you can download, print out and mail this Membership Renewal Form.
Please note: we recommend this Renewal Form be used only in case of lost or unreceived Renewal Notices.
